Canela Home Health Inc. is committed to protecting the privacy of your health information. This notice applies to the protected health information (“PHI”) we create and maintain in the course of providing home health care.

• Treatment. To provide, coordinate, and manage your care, for example, sharing information among our nurses, aides, and your physician.
• Payment. To obtain payment for the care we provide, such as billing and confirming coverage with a health plan.
• Health care operations. To support the business activities of our agency, such as quality assessment, staff training, and care review.

Other uses and disclosures may be required or permitted by law (for example, public health, health oversight, or as required by court order). Uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

Your rights regarding your health information

You have the right to:

• Inspect and obtain a copy of your health information.
• Request that we correct or amend your health information.
• Request an accounting of certain disclosures we have made.
• Request restrictions on certain uses and disclosures.
• Request to receive confidential communications by alternative means or location.
• Be notified following a breach of your unsecured protected health information.
• Obtain a paper copy of this notice upon request.

Our duties

We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future. Any revised notice will be made available on this website.

How to file a complaint

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (HHS OCR), 200 Independence Avenue, S.W., Washington, D.C. 20201, by calling 1-877-696-6775, or by visitinghhs.gov/hipaa/filing-a-complaint. You will not be penalized or retaliated against for filing a complaint.
