Common ways home care is paid for
Every family's situation is different, and the details below are general, not a promise of coverage. Use it as a starting point, then let's talk through the specifics together.
| Payer option | What it may cover | Good to know |
|---|---|---|
| Medicare | May cover eligible skilled home health, such as skilled nursing or therapy, for people who meet Medicare's criteria, including being considered homebound and under a doctor's plan of care. | Coverage is tied to specific rules and is time-limited. Non-medical personal care alone is generally not covered. |
| Medi-Cal | California's Medicaid program may help with certain in-home services for those who qualify financially and by need. | Programs and eligibility vary. Related programs like IHSS are administered separately from home health. |
| Medi-Medi (dual eligible) | For people who have both Medicare and Medi-Cal, the two may work together to cover more of the cost of eligible care. | Which program pays first depends on the service and your specific eligibility. |
| Private insurance / Medicare Advantage | Some private plans and Medicare Advantage plans include home health or personal-care benefits. | Benefits differ plan to plan, it's worth checking your policy or calling your plan directly. |
| Private pay | You can arrange and pay for care directly, which offers the most flexibility in scheduling and the type of help you receive. | Often used for non-medical personal care, companionship or hours beyond what a plan covers. |
This overview is for general information only and isn't insurance or legal advice. Coverage depends on your specific plan, eligibility and needs.

Coverage questions, answered
Does Medicare cover home health?
Medicare may cover eligible skilled home health services for people who meet its requirements, typically including being considered homebound and receiving care under a doctor's plan of care. Coverage is rule-based and time-limited, and non-medical personal care by itself is generally not covered. We can walk you through how the general rules might apply to your situation.
What's the homebound requirement?
"Homebound" is a Medicare term that generally means leaving home takes considerable effort and usually requires help or assistive devices, so trips out are infrequent. It does not mean you can never leave, short outings for medical care, worship or family events are typically fine. Whether someone meets the definition is decided case by case.

We're happy to talk through the basics and point you toward the right resources. Medi-Cal and programs like IHSS have their own eligibility rules and are administered separately, so we'll be honest about what we can help with directly and where an official program office is the best next step.
